Personal Attendant for Leisure Card

A PAL (Personal Attendant for Leisure) card allows a person with a disability to be accompanied by a support person so that they may participate in designated City of Thunder Bay programs and facilities as well as locally operated private businesses who have registered to be a PAL Card Acceptor (see below list).

The PAL card:

  • Identifies that the person named on the card requires the support of a PAL.
  • Authorizes that the PAL will not be charged a program/facility/service fee.

 PAL Card Acceptor List:

You are eligible if you meet the following two requirements:

  1. You are person with a disability that restricts you from participating in or performing activities. and,
  2. Your disability can NOT be eliminated by the use of a technical aid i.e. eyeglasses.

Temporary PAL cards can be issued to those with temporary disabilities.

How To Get Your Own PAL Card:

  1. Download, Print and Complete the application form (pdf format) 
  2. Fax/mail the application to the number/address at the bottom of the application
  3. Once the application has been approved, you will be contacted to make arrangements for a photo to be taken for your PAL card identification
  4. When you receive your card, you and your PAL are welcome to attend designated City of Thunder Bay programs
